Welcome to Remote Monitoring

Your provider has enrolled you in a remote monitoring program. This allows your care team to review your health readings between office visits and reach out if something needs attention.

IMPORTANT: This is not an emergency service

If you are having a medical emergency, call 911 or go to the nearest emergency room. This program is not monitored continuously, and we will not contact emergency services on your behalf.

How It Works

Take a reading using your monitoring device.

Your reading is sent automatically to your care team.

We review your readings regularly.

We will contact you if we notice any changes that need attention.

How Often Should I Check Readings?

Follow your provider’s instructions for how often to take readings. If you are unsure, see the back of this page for guidance specific to your device, or call the care team.

Your Device Is For You Only

Because readings from the device become part of your medical record, please do not share it with family members or others. If someone else uses your device by mistake, please let us know right away so we can exclude that reading from your chart.

Your monitoring device is provided by your clinic as part of your Connected Care program and is intended for your use while enrolled. If you ever leave the program, your care team will provide instructions regarding the return of any clinic-owned equipment.

Your provider may recommend one of these devices as part of your  Connected Care program.
